Speculation: He can memorize 2 minutes on a variety of topics, which does great for domestic issues. But on the foreign policy debate, which was mostly about Iraq, he had to keep saying the same things over and over and his poor ad lib ability was revealed.

Two explanations for why Bush can't remember that he previously dismissed the importance of Osama bin Laden
a) his memory is impaired --- this also explains why he cannot remember any mistakes he has made
b) his staff are as disconnected as he is, and cannot help connect him to reality because they are equally out of touch.
Could they actually have overlooked the liklihood that Osama Been Forgotten would be mentioned? Only if their reading has been limited to the certified right wing press (which explains lots of problems).
